The Death of Public Health

During the recent measles outbreak, the largest in more than 30 years, Robert F. Kennedy Jr. refused to clearly recommend the measles vaccine, saying instead that it was a “personal decision,” and that parents should “do their own research.” Similarly, during her recent confirmation hearing for United States Surgeon General, Casey Means was questioned by Senator William Cassidy (R, La). “You’re the nation’s doctor,” said Cassidy. “Would you encourage [a mother] to have her child vaccinated!” “I’m not an individual’s doctor,” Means said. “And every individual needs to talk to their doctor before putting a medication in their body.” In June 2025, Marty Makary, Commissioner of the Food and Drug Administration (FDA), said that COVID vaccines for pregnant women should be a “personal choice”, even though every other country in the world recommended the COVID vaccine during pregnancy due to a higher risk for severe disease. It doesn’t end there. On January 5, 2026, RFK Jr. reduced the number of routinely recommended vaccines from 18 to 10. For vaccines that protected against influenza, COVID, and rotavirus, parents were urged to speak with their doctors—to make an individual choice. (On March 16, 2026, in a ruling by a federal court in Boston, RFK Jr.’s revised schedule was invalidated.) What’s happening? Why are public health officials moving away from making recommendations for the public? The origin of this change might be rooted in the first two years of the COVID pandemic. In 2020, during the first year on the pandemic, we didn’t have anything to stop a virus that was killing hundreds of people a day and could be spread asymptomatically. We didn’t have antivirals until October, monoclonal antibodies until November, and vaccines until December. All we could do was restrict travel, close businesses, shutter schools, and mask, isolate, quarantine, test, and social distance. During the second year of the pandemic, when we had vaccines to prevent COVID, people couldn’t go anywhere without their vaccine cards. They couldn’t go to their favorite bar or restaurant or sporting event or place of worship without proof of vaccination. Some people were fired from their jobs for refusing to get vaccinated. For many, this was seen as massive government overreach and energized what has been called the “medical freedom movement.” Indeed, a headline in the New York Times declared, “How Health Freedom Became a Winning Rally Cry.” As expressed in the comments of RFK Jr., Casey Means, and Marty Makary, the medical freedom movement focuses on the individual not the community—the opposite of public health. In an article titled “Ethics and Social Value Judgments in Public Health,” ethicists from Yale University distinguished public health from medical freedom. “Public health policies,” they wrote, “are population oriented. Because individual health affects the health of others, public health measures regulate individual behavior to achieve population health goals. All raise questions about how individual autonomy and freedom should be balanced against public health interests…which respects individuals’ sovereignty over their bodies and actions as long as their actions do not harm others.” The medical freedom movement fails to recognize that it often limits the freedom of others. For example, a parent who is afraid to send a child to school where many of the students are unvaccinated. Or immune compromised children who depend on those around them to for protection. The question then becomes “Freedom for whom?” Is it your right, in the name of medical freedom, to catch and transmit a potentially fatal infection—and in so doing, take away the freedom of others? In Four Essays on Liberty, Isaiah Berlin sums it up best. “Liberty for the wolves,” he wrote, “is death to the lambs.” Thanks for reading Beyond the Noise! Forum on harnessing Artificial Intelligence for health equity

Register to this forumOver the past decade, artificial intelligence (AI) has significantly evolved in complexity, performance, and utility. Its integration across a wide range of industrial sectors has accelerated, with healthcare emerging as one of the most transformative sectors. In this sector, AI has enabled high-impact applications – from enhancing the speed and accuracy of disease diagnosis and supporting clinical decision-making, to strengthening public health initiatives such as disease surveillance and workforce optimization. Moreover, AI holds immense potential to empower individuals, enabling them to take greater control of their personal health and well-being. Yet the central question remains: Who benefits, who is at risk, and do these innovations close or widen health inequities? While the potential of AI-enabled applications is substantial, many countries have approached their adoption with caution. This hesitancy stems from concerns about risks and benefits, including model bias, data breaches, privacy violations, and the erosion of professional skills due to over-reliance on AI. Additional barriers include inadequate data infrastructure and a lack of robust evidence on the most effective forms and interfaces of AI to enhance service delivery and system performance. Moreover, the rapid rise of Generative AI has introduced a heightened urgency for the development of comprehensive governance and ethical frameworks. The Asian Development Bank (ADB) Health Practice Team and Digital Sector, in collaboration with the World Health Organization (WHO) Regional Office for the Western Pacific, are convening the Forum on Harnessing AI for Health Equity on 25–26 March at ADB Headquarters. Senior health officials from 16 developing member countries (DMCs), alongside leading researchers, innovators, and policymakers from across Asia and the Pacific, will engage in discussions on how AI can be governed, financed, and scaled to deliver healthier lives for all. The forum is funded through the High-Level Technology Fund with support from the Government of Japan. Objectives The two-day forum aims to:Identify high-impact, evidence-based health AI use cases suitable for resource-constrained settings and map out a pipeline for potential supportIntroduce and validate the WHO's policy guidance on minimum necessary ecosystem for countries and institutions to self-assess readiness for AI adoptionObtain initial commitment from participating stakeholders to join a pilot regional initiative focused on data/model sharing for a priority health challengeEndorse a prioritized set of actionable policy recommendations focusing on governance, regulation, and sustainable financing necessary to scale high-impact health AI use cases in resource-constrained settings Target participants The forum will convene participants from 16 ADB DMCs including Bangladesh, Bhutan, Cambodia, Cook Islands, Fiji, Indonesia, Lao People's Democratic Republic, Mongolia, Nepal, Palau, Papua New Guinea, Philippines, Sri Lanka, Thailand, Uzbekistan, and Viet Nam. In addition, health system leaders and practitioners, private sector technologists, academic experts, representatives from civil society organizations, development partners, and philanthropic institutions engaged in AI for Health from Australia, the People’s Republic of China, Japan, Malaysia, New Zealand, Singapore, and the Republic of Korea will also be invited. Strengthening One Health Field Epidemiology Capacity in the WHO South-East Asia Region

Recent public health emergencies—including the COVID-19 pandemic, recurrent zoonotic disease outbreaks, and the high level of antimicrobial resistance—have underscored the urgent need for strong, multisectoral field epidemiology capacities. This reinforces the importance of a workforce equipped with One Health field epidemiology competencies. To address this need, WHO works with the Food and Agriculture Organization of the United Nations (FAO), the United Nations Environment Programme (UNEP), and the World Organisation for Animal Health (WOAH), as part of Quadripartite Alliance for One Health, to strengthen field epidemiology training programmes using a One Health approach. These efforts are aligned with WHO SEARO’s Strategic Framework for Action for Strengthening Surveillance, Risk Assessment and Field Epidemiology and the Regional Roadmap to Advance Field Epidemiology Capacities 2025–2029 emphasize workforce development, multisectoral collaboration, and enhanced preparedness for health emergencies. With funding from the Pandemic Fund, The Competency for One Health Field Epidemiology (COHFE) integration within Field Epidemiology Training Programmes (FETPs) or similar initiatives has been initiated in Bangladesh and India and is planned in Sri Lanka.BangladeshIn Bangladesh, COHFE is being adapted to strengthen the Field Epidemiology Training Programme, Bangladesh (FETP-B) Advanced programme. Initiated in July 2025 and led by the Institute of Epidemiology, Disease Control and Research (IEDCR), the process brought together stakeholders from human, animal, and environmental health sectors, academia, and laboratories.A Multisectoral Working Group (MWG) was established to guide implementation, conduct readiness assessments, and identify opportunities to institutionalize One Health collaboration. Through national multisectoral consultations and a competency prioritization workshop held in Dhaka in November 2025, stakeholders identified priority technical competencies for surveillance, outbreak investigation, data analysis, and risk assessment, alongside functional competencies related to leadership, coordination, and the laboratory–epidemiology interface. The process also surfaced systemic barriers—such as challenges in data sharing and limited joint investigations—enabling targeted recommendations. These outcomes are guiding curriculum revision, the introduction of One Health simulations and joint field exercises, and the development of a national multisectoral mentorship pool, with updated training materials expected to be implemented in the upcoming cohort.Figure 1. Competency prioritization involving multisectoral stakeholders“COHFE represents a significant step forward in operationalizing the One Health approach. The collaboration between national institutions and the Quadripartite partners is critical for building resilient systems to address complex health challenges in the region.”, Prof. (Dr) Tahmina Sirin, Director, Institute of Epidemiology, Disease Control and Research (IEDCR), Bangladesh.Aligning India’s One Health Training with COHFEIn India, the National Centre for Disease Control (NCDC) has strengthened One Health workforce development through its SectorConnect initiative. WHO supported a comprehensive competency mapping and curriculum review to align the Field Epidemiology Programme in One Health (FEP-OH) with all 14 COHFE domains. The review identified strong foundations while generating actionable recommendations to strengthen One Health learning outcomes, expand joint field-based learning, and enhance competency-based assessment. These improvements are informing programme refinement and phased scale-up across additional states.Figure 2. Strengthening learning outcomes for the SectorConnect FEP OH CurriculumThrough these efforts, WHO SEARO will continue to support countries to move from separate sector-based training toward an integrated, impact-driven One Health field epidemiology workforce capable of addressing complex health threats at human-animal-environment interface.ReferencesFood and Agriculture Organization of the United Nations, World Health Organization, & World Organisation for Animal Health. PRET for impact: Advancing pandemic preparedness in the Democratic People’s Republic of Korea

IN THE SPOTLIGHT PRET for impact: Advancing pandemic preparedness in the Democratic People’s Republic of KoreaThe Democratic People’s Republic of Korea (DPR Korea) is making significant progress in strengthening pandemic preparedness, with dedicated support from the WHO South‑East Asia Regional Office (SEARO) and the Pandemic Influenza Preparedness (PIP) Partnership Contribution (PC).Aligned with the WHO Global Influenza Strategy and the Pandemic Influenza Preparedness (PIP) Framework Partnership Contribution High‑Level Implementation Plan III (2024–2030), the country is strengthening its capacity to prepare for and respond to influenza and other respiratory threats. In early 2025, a core team from the Ministry of Public Health of the Democratic People’s Republic of Korea (DPRK) participated in a two‑day virtual workshop, which included a mini‑simulation exercise. The workshop was organized by WHO SEARO in collaboration with the Preparedness and Resilience for Emerging Threats (PRET) Secretariat at WHO headquarters and the WHO Country Office for the DPRK. Read more A REGIONAL CONSULTATION GUIDES WHO SEARO IN DEVELOPING A ROADMAP FOR IMPLEMENTING THE PREPPAREDNESS AND RESILIENCE FOR EMERGING THREATS (PRET) INITIATIVE BASED RESPIRATORY PATHOGEN PANDEMIC PREPAREDNESSInfluenza and other respiratory pathogens with pandemic potential continue to pose major public health risks globally. To strengthen regional preparedness and response, Member States of the WHO South‑East Asia Region (SEAR) convened in Colombo, Sri Lanka, to review and provide input on the draft Roadmap and Action Framework for Broad Respiratory Pathogen Pandemic Planning (2026–2031). Supported through the Pandemic Influenza Preparedness (PIP) Partnership Contribution (PC) funds, the proposed roadmap aims to advance robust, integrated preparedness and response capacities for influenza and other respiratory threats across SEAR. This work is aligned with the Preparedness and Resilience for Emerging Threats (PRET) initiative and the SEAR Strategic Roadmap on Health Security and Health System Resilience for Emergencies Read moreSRI LANKA’S USE OF ANTIVIRALS TO REDUCE THE SEASONAL INFLUENZA BURDEN Sri Lanka is strengthening its influenza response by placing greater emphasis on rapid testing, timely treatment, and integration with maternal health services. With support from WHO, the country is building a stronger evidence base on the burden of influenza and the value of vaccination, advancing a holistic approach to protect vulnerable populations and enhance pandemic preparedness. To reduce hospitalizations and deaths from seasonal influenza, Sri Lanka is taking proactive steps to expand access to rapid diagnostic testing and ensure prompt initiation of treatment. National clinical guidelines recommend the use of oseltamivir for individuals at high risk of complications, helping prevent severe illness and shorten the duration of infection. This strategy not only saves lives but also supports local pharmaceutical production, contributing to a reliable supply of antivirals for both seasonal outbreaks and future pandemics.Read moreBHUTAN BOOSTS PANDEMIC INFLUENZA READINESS WITH PANDEMIC INFLUENZA PREPAREDNESS FRAMEWORK PARTNERSHIP CONTRIBUTION SUPPORTED TRAINING AND SIMULATIONSOriginally developed in 2012, the National Influenza Pandemic Preparedness and Response Plan (NIPPRP) has long served as Bhutan’s central framework for managing influenza threats. In 2024, the plan underwent a comprehensive update to reflect evolving global and regional guidance, lessons learned from the COVID‑19 pandemic, and new capacities developed under the PIP Framework. The revised NIPPRP is now more clearly aligned with Bhutan’s broader emergency and disaster risk management structures, ensuring a coordinated, whole‑of‑government approach to pandemic response. As part of efforts to strengthen national pandemic preparedness, Bhutan conducted three national‑level training sessions followed by simulation exercises on influenza pandemic preparedness and response at five sentinel hospitals between April and May 2025, with support from the PIP Partnership Contribution (PC). These activities brought together more than 140 health professionals and key partners from across the country, with a focus on operationalizing the updated NIPPRP and enhancing readiness at key frontline facilities. Originally developed for influenza and adapted during COVID-19, Unity Studies enable countries to conduct rapid, nationally led investigations and studies to assess transmissibility, severity, susceptibility, and intervention effectiveness. To better prepare for future influenza pandemics, WHO has recently launched a network of sites which will be ready to conduct these investigations and studies rapidly in the event of a pandemic, or epidemic of an emerging or re-emerging respiratory pathogen. The WHO South-East Asia Region now hosts six sites of WHO GISRS Unity Studies across the five countries including Sri Lanka, strengthening regional capacity for early pandemic investigations. From 13–16 July 2025 in Colombo, the Global Public Health Summit offered an opportunity to advance this agenda. Read moreSTRENGTHENING PANDEMIC PREPAREDNESS THROUGH UNITY STUDIES: REAL-WORLD APPLICATIONS FROM INDIA IN WHO’S SOUTH-EAST ASIA REGION As countries continue to strengthen their pandemic preparedness through investments in resilient health systems during the interpandemic period, the World Health Organization’s (WHO) Global Influenza Surveillance and Response System (GISRS) Investigations & Studies Global Initiative, Global Initiative, commonly known as Unity Studies, remains a vital framework for respiratory pathogen pandemic preparedness. These standardized epidemiological protocols enable countries to rapidly generate high‑quality data on transmissibility, infection severity, population immunity, and the effectiveness of public health interventions. To highlight how Unity Studies protocols are being adapted and applied in national settings, WHO Headquarters, together with WHO Regional Offices, including the WHO South‑East Asia Regional Office (SEARO), hosted a global webinar. United States and Madagascar to Implement $175 Million Five-Year Health MOU - U.S. Embassy in Madagascar

United States and Madagascar to Implement $175 Million Five-Year Health MOU U.S. and Malagasy health officials are convening in Antananarivo for a two-day joint review process, marking the first major step in implementing the strategic health cooperation framework established in the Memorandum of Understanding (MoU) signed between the two countries in December 2025. The MoU outlines the structure and terms of U.S. support to Madagascar’s health sector from 2026 through 2030, with a total commitment of $134,305,000 from the U.S. Government and $41,216,252 from the Government of Madagascar working jointly to address malaria, maternal and child health, polio and global health security issues. This approach reflects the America First Global Health Strategy by prioritizing effective, results-driven cooperation that advances mutual health security and supports Madagascar’s progress toward self-reliance and independence from foreign assistance. The events bring together U.S. Government health experts and Madagascar’s Ministry of Public Health leadership to conduct a comprehensive review of seven major health programs currently supported by United States funding, discussing current activities, and developing recommendations for programs to align with the signed Memorandum of Understanding, which advances both the America First Global Health Strategy and the Government of Madagascar’s goals. “This memorandum sets the course for U.S. strategic cooperation and support to Madagascar’s health sector over the next five years,” said Chargé d’Affaires Stephanie Arnold at the workshop’s opening. This review will help determine how to maximize the effectiveness and impact of these programs, ensuring resources provided by U.S. taxpayer dollars are used wisely,” she added. “By working together to address global health challenges, we demonstrate America’s leadership and commitment to a safer, healthier world, making both the United States and Madagascar healthier and stronger.” Under President Trump’s leadership, the United States is implementing a Global Health Strategy for foreign assistance that emphasizes increased host country ownership, resilient health systems and human resource capacity to reduce dependency, maintaining frontline support to reduce infectious disease threats, and strong performance monitoring systems for accountability and service delivery. “Diseases do not respect borders, and when we support Madagascar’s capacity to detect, prevent, and respond to infectious disease threats, we are contributing to global health security that benefits communities both here and in the United States,” Arnold emphasized, noting that “The MoU represents a commitment to transparency, accountability, and results.” The United States recognizes the commitment of Malagasy leadership in prioritizing investment of time and significant resources on the healthcare of their own citizens. With recent changes in leadership, Madagascar has an opportunity to strengthen governance, improve performance, and move forward toward greater self-reliance, particularly in the healthcare sector. Through strategic focus and thoughtful collaboration under this MoU, U.S. health programs in Madagascar will deliver meaningful results that advance both countries’ interests while supporting Madagascar’s journey toward greater self-reliance and a healthy future.

Has the vegan business bubble burst? : It's Been a Minute

Is veganism in its flop era? There was a time when veganism – or plant-based food – seemed to be in ascendency: from the early 2010s when Beyoncé adopted a plant-based diet to its height in 2020, when the retail market for plant-based meat made over a billion dollars in sales. And then… things started getting a little more… meaty. From protein-maxxing to the new food pyramid, the culture seems to have shifted. But what does the plant-based food flop era say about our culture, how we think about our food, and how we feel about our future? Special guest host Danielle Kurtzleben, NPR political correspondent, discusses with writer Rachel Sugar, who wrote a piece on the boom and bust of veganism for New York Magazine, and Mark Bittman, long-time food journalist and former New York Times columnist who has written about veganism since the early 2010s.Support Public Media.
