In Good Health: The Matter Of Maternal Mental Health Care

The Lindsay Clancy murder trial captivated the American public for weeks.Prosecutors charged Clancy with murdering her three children. They say she then tried to take her own life and that her injuries left her paralyzed from the waist down. Clancy’s lawyers say she was experiencing postpartum psychosis and should not be held criminally responsible. But prosecutors say she planned the deaths. After seven days of deliberation, the jury remained deadlocked and the case ended in a mistrial. The trial has divided the nation and sparked conversations over about maternal mental health, raising questions over gaps in care for parents after giving birth. About one in five expecting or postpartum women in the U.S. experience mental health disorders. And according to a 2025 study, overall maternal mental health has dramatically dropped in the past seven years, with only 25 percent of moms reporting excellent mental health. What do we know – and don’t know – about maternal mental health care? And how can our systems be bolstered to help those who need it most after giving birth? If you are experiencing a mental health crisis or thinking of harming yourself, the 988 Suicide and Crisis Lifeline is available 24/7 by phone and text. The National Maternal Mental Health Hotline is also available 24/7. You can text, call or chat at 1-833-TLC-MAMA. Postpartum politics: What it means to run for government as a new mum

Victorian Greens candidates Nina Crawley and Lini Fernando reflect on their experiences running a political campaign just months after giving birth. From the challenges and compromises to the support networks that wrapped around them, and why showing up vulnerably in public space matters. Subscribe In 2024, when my second child was four months old, I decided to run for local government. Fast forward two years, I am again four months postpartum, with my third child, and this time running in the state election. But this time has hit differently. Going through pregnancy, birth and postpartum while being a political candidate has had a more profound and personal impact than I ever expected. I reached out to my fellow Greens candidate for the state election, Lini Fernando, who gave birth 20 days before me, and her experience closely mirrored mine. Postpartum has its own unique challenges that we’ve not seen reflected in stories about women running for office. When people discuss the “juggle”, they’re usually talking about the practical mechanics of looking after small children - childcare arrangements, pumping schedules and logistics. As mums of multiple children, we were both aware of the logistical changes and difficulties that accompany giving birth and the months after. We assumed that combining this with running for election would be hard, but that we could organise our way out of any problems. What neither of us anticipated was the intense vulnerability that comes with putting yourself into the spotlight of a state election campaign at a time when you are physically exhausted and navigating things like breastfeeding and feeling like a stranger in your own body. There have been challenges that neither of us anticipated. Firstly, needing to buy new clothes because the supportive leggings that are a staple of the postpartum wardrobe weren’t going to cut it for community events. Lini held her first fundraiser - a trivia night - just three weeks postpartum. A business-casual wardrobe, several sizes bigger than pre-baby, was needed, and fast. As women, how we look is closely scrutinised, and the internalised fatphobia is loud. We are conditioned from our earliest days to believe that our appearance is tied to our worth, and taught how we are allowed to show up in public. Women who dare to put themselves into the spotlight also receive more abuse than their male counterparts, including criticism of their appearance. To stand in front of members of your community and try to convince them that you’re the best person to represent them in parliament is a challenge in and of itself. To do it with leaking breasts, a pelvic floor that could betray you at the first sign of a sneeze, a body that still feels unfamiliar, and with half your hair falling out (hello postpartum hair loss!) is another thing altogether. It requires courage, allowing yourself the grace to be vulnerable in public, and the belief that showing up, however imperfectly, matters. And the vulnerability isn’t just physical; it also includes factors like having to ask a campaign team to accommodate the realities of having a baby. Asking for help is hard, and it’s a skill that both new parents and hopeful political candidates need to become comfortable with. Parents understand the disappointment that comes when a call for help or connection is rebuffed, like a friend cancelling a much-needed coffee. That rejection is amplified for political candidates calling dozens of people each week. Many people don’t realise running for Parliament, when you aren’t the incumbent, is a volunteer role supported largely by other volunteers who have to squeeze volunteering in around their paid work. Calls need to be made and meetings held after work hours - usually during “witching hour” (5-8 pm), that special time of night when babies cry for no reason. I found that I was struggling to make these commitments, or having to tap out and ask for them to be rearranged. When I spoke to Lini, we realised that she had had a similar experience, this push-pull of being the candidate and wanting to show our faces at meetings and help recruit new volunteers at ‘calling parties’, but also being the only people who could settle our newborns. Sometimes we could merge the two worlds; Lini and I realised that we had both attended 7:30pm meetings online, sitting in a dark room, struggling though breastfeeding and settling our babies. Although we’re both committed to breaking the stigma around breastfeeding, it can be hard to keep your composure while trying to feed a young baby as they try to latch, spew and fuss. Ultimately, Lini and I agreed that showing up vulnerably in public space matters. Leaders should possess empathy and understanding for those who feel raw and in transition; from the dad who just got evicted to the teen who acquired a disability - anyone who feels they aren’t the version of themselves they expected right now. The patriarchy tries to tell us otherwise, but women shouldn’t have to wait until they’re ‘recovered’, no longer vulnerable, before they feel entitled to participate in public life. In fact, we could all benefit from our leaders being a little more vulnerable. *This piece was cowritten, but has been presented in the first person for the sake of style. Clinton Policy Institute outlines maternal health solutions for states

Health Care Clinton Policy Institute outlines maternal health solutions for states It’s the first major report from the project, which launched in July to foster nonpartisan policies to improve the wellbeing of children and families. (AP Photo/Mary Conlon) The Clinton Policy Institute is calling on states to lead efforts to curb maternal mortality, releasing a 61-page playbook Thursday highlighting successful policies that could be adopted nationwide. The playbook is the first major report from the Clinton Foundation’s project, which launched in July to foster nonpartisan state and local policies to improve the wellbeing of children and families. “Eighty percent of maternal deaths are preventable, and we know what works,” CPI Chair Jennifer Klein said. “The solutions are well established and evidence-based.” CPI’s mission is to convene political leaders from both parties, policy experts, state officials and other stakeholders to explore solutions to longstanding problems. “What we aim to do is bring together people who are deeply engaged, but may not be meeting together in the same room,” Klein said, noting that state policymakers might not always be aware of what their peers are doing in other states. In May, CPI brought together lawmakers from Virginia and Pennsylvania, two of the 14 states that have passed bipartisan maternal health “Momnibus” packages modeled after a comprehensive federal proposal that has repeatedly stalled, called the Black Maternal Health Momnibus Act. Other participants included Dr. Mona Hanna, founder of Rx Kids, a Michigan program that provides direct cash allowances to pregnant women and infants. Pennsylvania Rep. Morgan Cephas, who was part of the group, said she learned about funding opportunities she hadn’t come across during her decade in the Legislature. “As legislators we can issue grants, we can pass legislation, we can move policy, but we want to ensure that funding opportunities and programs that are tackling this issue are sustainable,” Cephas said. The playbook outlines broad areas where state policy can make a difference, such as allowing Medicaid to pay for doulas and remote patient monitoring, and using federal and private grant funding to pay for home visits to pregnant and postpartum women. For each category, it highlights innovative programs that can serve as models, with examples of specific actions state lawmakers and governors’ offices can take. Those include the Rx Kids model, which gets funding from the Michigan state budget; Pennsylvania’s Healthy Moms Vibrant Futures strategic action plan, emphasizing equity-centered maternal care, regional collaboration, Medicaid innovation, and workforce development that was launched this year with more than 160 stakeholders; and Kansas’s Bridging Plan, which reserves three slots and provides at least $26,000 in loan forgiveness for obstetric residents who commit to practicing in rural or underserved communities for 36 months. CPI emphasizes that its work is nonpartisan. Klein, the director of the White House Gender Policy Council under former President Joe Biden, acknowledged that CPI also takes positions on abortion, Medicaid funding, and the need to address the economic and social drivers of poor health, which might rankle some conservatives. “We don’t shy away from talking about those things that we think are part and parcel of the maternal health crisis in this country,” Klein said. “That said, there are so many policies that are both evidence-based and have been tried successfully in both red and blue states, and people can see that they can have real impact.” She pointed to the federal option to extend postpartum Medicaid coverage from 60 days to 12 months that was made available during the Biden administration and has since been adopted by every state but Arkansas. Other initiatives embraced across party lines include Medicaid coverage for midwives and doulas, as well as “bundled” payments for prenatal and postpartum care. That alternative payment model, pioneered in states such as Arkansas, Colorado and Tennessee, incentivizes fixed fees to cover all healthcare services associated with pregnancy and childbirth. Advocates say it promotes more coordination among caregivers, discourages unnecessary interventions, and expands postpartum support. In addition to maternal health, the initiative is also focused on addressing family economic security, children’s well-being, and democracy and civic resilience.
