How to Manage Symptoms of Heart Failure
(Feature Impact) Americans are facing a growing health challenge, with 1 in 4 adults at risk of heart failure, according to the Heart Failure Society of America. Additionally, approximately 6.7 million U.S. adults who are 20 years old or older are currently living with heart failure, and that number is projected to reach 8.7 million by 2030. googletag.cmd.push(function() { googletag.display('JGFI_Article_300x250'); }); Managing heart failure is a significant expense to the U.S. healthcare system, costing the nation approximately $40 billion in 2020 and expected to increase to approximately $142 billion a year by 2050.If you (or a loved one) are living with the condition, consider these tips to help manage it.Adhere to the Treatment and Follow-Up PlanOngoing outpatient care is important when managing heart failure. Understand the treatment plan provided by your healthcare team, make diet and lifestyle changes, take medications as directed and keep recommended follow-up appointments.Pay attention to changes in symptoms and communicate with your healthcare provider if treatment does not seem to be working as expected. If their conditions are left unmanaged, patients can experience a range of heart-failure-related symptoms such as shortness of breath, fatigue, buildup of fluid and more.Understand Challenges with Diuretic TreatmentDiuretics help the body remove excess fluid and are commonly used to treat swelling, or edema, associated with heart failure. Your provider may try different diuretics, sometimes referred to as “water pills,” to manage your heart failure symptoms. However, intravenous (IV) diuretic therapy may be required for some patients if symptoms continue.Ask About Treatment Options Before IV Care Is NeededFor some adults, a healthcare provider may consider recommending an at-home, non-oral diuretic option prior to escalating to IV therapy. For example, ENBUMYST™ (bumetanide nasal spray) is a prescription diuretic used in adults to treat edema associated with congestive heart failure. It’s administered as a nasal spray for short-term use to manage excess fluid with the goal of transitioning to oral diuretic therapy when appropriate.Talk with your healthcare provider to learn more and visit enbumyst.com. Read the full Prescribing Information and Patient Information.Photo courtesy of Shutterstock googletag.cmd.push(function() { googletag.display('JGFI_Article2_300x250'); }); Important Safety InformationWhat is ENBUMYST?ENBUMYST is a prescription medicine used in adults to treat swelling caused by excess fluid in the body (edema) from congestive heart failure, liver disease, and kidney disease. What is the most important information I should know about ENBUMYST?ENBUMYST is a diuretic that helps your body remove fluid. Losing too much fluid can cause dehydration, low blood volume, and increase the risk of blood clots. Blood abnormalities may include decreased salts (electrolytes), and increased nitrogen, glucose and uric acid. The risk of these abnormalities is higher in elderly people, those using higher doses, or those who do not get enough electrolytes by mouth. You should not use ENBUMYST if you have severe electrolyte loss. Tell your healthcare provider if you experience unusual thirst; dizziness or lightheadedness; weakness or tiredness; fast heart rate; dry mouth or tongue; sudden pain; redness or swelling in your joints; pain, swelling, redness, or warmth in your leg; shortness of breath or fast breathing; high blood sugar; dark urine; pale, tight, cool or clammy skin; muscle cramps; confusion.Do not use ENBUMYST if you have low or no urine production or are allergic to bumetanide or to any of the ingredients in ENBUMYST.Before using ENBUMYST, tell your healthcare provider if you have low electrolytes, are on a low-salt diet, have diarrhea, diabetes, nasal problems such as a runny or stuffy nose or polyps, a history of nasal injury or surgery, are or plan to become pregnant, are or plan to breastfeed.Tell your healthcare provider about all the medicines you take, including prescriptions, over-the-counter medicines, vitamins, and herbal supplements.ENBUMYST may cause serious side effects, such as worsening kidney function and ringing in the ears or hearing loss. The most common side effects of ENBUMYST include dehydration, headache, muscle cramps, dizziness, low blood pressure, nausea, and abnormal brain functioning with certain liver problems. These are not all the possible side effects of ENBUMYST. Regional Science Consortium Researching Harmful Algal Blooms
If you've noticed large buoys or boats out on the bayfront and wondered what they were, you're not alone. The Regional Science Consortium is collaborating with Princeton Hydro on a research project funded by the U.S. Army Corps of Engineers to study ways to control harmful algal blooms. Dr. Jeanette Schnars, the Executive Director of the Regional Science Consortium, explained the goal of the research."We are researching different ways to control blue-green algae, which creates harmful algal blooms and releases toxins," Dr. Schnars said. The project aims to identify potential techniques for controlling harmful algal blooms in Presque Isle Bay.Schnars explained that people may notice circular floats in the water near Perry's Landing and Liberty Park. Those floats are part of the testing process."You might see a circular float, almost like a hula hoop, that is out in the bay, near Perry's Landing and Liberty Park area, and those limn corrals go all the way to the bottom. And what we will do are some treatments within the limn corral in the bottom," Schnars said. After samples are collected, they are brought back to the laboratory at the Tom Ridge Environmental Center for analysis."A lot of those samples are coming back here. We are going to test them for the harmful algal bloom toxins before, during, and after, and see how effective the testing was," Schnars said.Sarah Magyan, an aquatic biologist with the Regional Science Consortium, explained that the team also collects samples from locations throughout the region."We pull samples every week from 30 different sites, and then we bring those back here, we preserve them and run them through the ELISA test," Magyan said.Schnars said researchers hope the results will help them develop effective ways to reduce harmful algal blooms in the future."If we can do some treatments over space and time in upcoming seasons, we may be able to reduce the number of harmful algal blooms we have in this area," Schnars said. The research is also important for public and animal safety. Magyan explained that toxins produced by certain algae can affect the skin, nervous system or liver. While toxin levels may be low for people, she emphasized the importance of protecting pets that swim in local waterways."Toxins can either be skin toxins, nerve toxins, or liver toxins. While we have low levels for people, it is important that we are keeping our pets safe because we recreate with our animals in the water, and we see hundreds and hundreds of dogs on Presque Isle and the rest of the shoreline," Magyan said.Testing began in September, and Schnars said researchers expect to conduct at least another six to eight weeks of testing to better understand the effectiveness of the treatments.

Concern after Europeans endorse US science agenda
Germany’s biggest research funder has said it is “concerned” after Berlin, plus several other European countries, endorsed a controversial US-led science policy statement. On October 4, the White House announced that 16 countries, including Germany, the UK, Italy, Greece, Bulgaria, Cyprus, Poland, Japan and South Korea had backed the Kyoto Vision for a Golden Age of Science. The document, agreed on the margins of the annual Science and Technology in Society forum in Kyoto, which gathers science ministers from across the world, appears uncontroversial at first sight. It’s a one-page statement, without any concrete measures, that touches on reducing administrative burden on scientists, experimenting with new funding models and the use of “super intelligence” in science, the Trump administration’s preferred term for artificial intelligence. However, what’s controversial is that much of the language in the Kyoto Vision reflects Science: A New Golden Age, a major policy document released by the White House in July. That document attracted criticism for deriding universities as “legacy institutions” and suggesting money be diverted to new research labs. In a speech in Kyoto on October 4, Michael Kratsios, director of the White House Office for Science and Technology Policy and the report’s author, said that the US was seeking to “diversify the ways that we support research to meet our best scientists where and when they do their best work.” The worry is that the Trump administration may be using genuine dysfunctions in academia as a pretext to defund and disempower universities, which US vice president JD Vance has labelled “the enemy.” Many in Europe also want more experimentation and reform of how science is organised and funded, and agree with criticisms of academia made by Science: A New Golden Age. But some also fear that if the US pushes for reform partially in bad faith, it could politicise and undermine the whole agenda. Washington is currently trying to change federal funding rules so that political appointees can block and cancel scientific grants. It has cut grants mid-project on topics related to diversity and other politically forbidden areas. And last year, it threatened to cut Harvard University’s federal funding unless it admitted a “critical mass” of new faculty and students to create campus “viewpoint diversity.” “Particularly concerned”Because of this wider US political context, the Kyoto Vision’s otherwise innocuous sounding statement that “we encourage experimentation with new institutional models for science that provide varying levels of autonomy, resources and direction” has caused consternation in Europe, particularly from some of Germany’s biggest science organisations. A spokesperson for the German Research Foundation (DFG), the country’s biggest funder, said that it would “closely monitor whether the current US administration uses the Kyoto Vision to restructure research funding to the detriment of research institutions such as universities and their autonomy.” The DFG was “particularly concerned” with the statement’s references to Science: A New Golden Age, they continued. A spokesperson for the German Rectors’ Conference, which represents the country’s universities, said that the reference to “new institutional models” raises “questions on the underlying understanding of academic freedom and autonomy.” Research funding must “guided first and foremost by scientific not political principles,” they went on. Dampen criticismEuropean endorsement of the Kyoto Vision might dampen domestic criticism of the Trump administration’s science policy, some in the US fear, by suggesting international support. “What we saw from Germany, Japan, the UK and other countries, is an impulse that the administration has exploited with our own science reform movement, a belief they can use dialogue to make things better,” said Cole Donovan, director of science policy at the campaigning group Stand Up for Science. “In practice, the administration has done the opposite.” Not a binding statementThere’s been so much concern in Germany that the country’s federal Ministry of Research, Technology and Space put out a statement defending its endorsement of the Kyoto Vision, which it has posted on its website. Related articlesIs metascience being co-opted?European researchers sound the alarm that Trump rule changes will hit cooperationThe US ‘golden age’ science strategy: what Europeans need to know The ministry said that it “should be regarded as a basis for discussion and does not constitute a binding statement.” Backing the statement gives Germany the chance to raise “issues and concerns” with the US during upcoming talks on science and technology scheduled for November. “Although these intentions appear, in principle, to be worthy of support, they will be scrutinised and examined more closely as the process continues,” the statement continues. Super intelligence?A ministry spokesperson confirmed to Science|Business that Germany had broadly endorsed the Kyoto Vision. However, they said Berlin had not agreed to final changes in the statement to refer to AI as super intelligence. In September, facing protests against data centres and polling that suggests US citizens are far more concerned than excited by the technology, Trump issued an executive order that attempts to rebrand AI as super intelligence, following polling on social media. The name change is to “reflect the transformative capabilities of these technologies and the limitless opportunities they create for the American people,” the order says. EU askedThe UK and Italian science ministries did not respond to a request for comment on their endorsement of the Kyoto Vision. Although most EU countries did not endorse the statement, it’s not clear which states the US asked. The White House Office for Science and Technology Policy did not return a request for comment. A European Commission official, speaking under condition of anonymity, said that the US had approached the EU, but too late for the EU to quiz all its member states on whether Brussels should endorse the statement.
