Whether you like it or not, celebrities have an impact on politics
The global groundswell of support for Macklemore’s pro-Palestinian advocacy serves to remind us that maybe celebrity activism really is that deep after all, writes Cheek’s Varsha Yajman. Subscribe In the last week, publications, podcasts, and influencers that have either rarely or never spoken about Israel’s genocide in Palestine have flooded my social feed. And it was all because singer Ed Sheeran and his team dropped Macklemore as his opener for saying “Free Palestine” on stage. Millions raised, millions speaking up in support of Palestine - all of this confirming my view that, whether I like it or not, celebrities do have an impact on political outcomes. And this is interesting when much of the time celebrity activism can induce an eye roll - I love to see Charli XCX pointing out a Palestinian flag in the crowd, but it’s hardly the bold symbol of allyship that some fans would make it out to be. But there’s a line between healthy scepticism and outright dismissal - and I’d argue that the latter lets the rich and powerful off far too easily. For instance, actress Jennifer Lawrence, once outspoken about her political views, last year told The Times she no longer believes celebrities have a role to play: “Celebrities do not make a difference whatsoever on who people vote for. So then what am I doing? I’m just sharing my opinion on something that’s going to add fuel to a fire that’s ripping the country apart.” While it’s a narrative she might find comfort in, it simply is not true. Grassroots movements often garner worldwide attention when celebrities and other high-profile spokespeople bring a mainstream platform to their causes. And this frustrates me. We have had three years of a live-streamed genocide. Macklemore himself has been calling for a Free Palestine long before he booked the Sheeran concert. Yet it’s the high-profile fallout of the last week that finally seems to have made saying “Free Palestine” socially acceptable in the mainstream. It feels like the collective mood has shifted, where calling out censorship is now the obvious response - not just the business of ‘radical’ activists. To recap, Sheeran’s decision followed an ultimatum from billionaire Robert Kraft, owner of Gillette Stadium, one of the venues Sheeran was scheduled to perform at: either drop Macklemore as your opener after only two of the eight scheduled shows, or you do not play at any stadiums. Sheeran’s disappointing decision to stand on the side of billionaire-led censorship has ultimately seen a surge in support for Free Palestine. It began when all his openers and his backing band pulled out of Sheeran’s tour, in solidarity with Macklemore and with the Palestinian cause. It was met with an uproar of support from those who had never or barely touched politics before: influencers lip-syncing to Macklemore’s music, singers like Jack Antonoff singing “Free Palestine,” and publications and podcasts standing with Macklemore. And finally, we saw material support: Macklemore donated $1 million in profits from his two nights opening for Sheeran’s tour to several Palestinian aid groups and called for Sheeran and Kraft to match it, which they have since promised to do. We’ve also seen children’s entertainer Ms Rachel follow suit with a $1 million donation to Palestinian aid, and publication PopBase has donated $10,000. Anera, a humanitarian aid group working in Palestine, Lebanon, and Jordan, said donations since Macklemore’s announcement were more than triple this month’s daily average. The group’s Instagram page has also gained 8,000 followers. Macklemore far from the first celebrity to speak and participate in politics in the Western World. In 1924, singer Al Jolson supported Republican candidate Warren G. Harding for president, and decades later, celebrities such as Harry Belafonte, Lena Horne, and Aretha Franklin voiced their public support for the Civil Rights movement. More recently, in the lead-up to the 2024 US election, ‘The Daily Show with Trevor Noah partnered with the organisation Power the Polls to recruit poll workers to address shortages- a problem disproportionately affecting voters of colour. The campaign saw 170,000 new sign-ups - 35,000 of which came directly through the TV show’s link - resulting in the recruitment of 713,000 total poll workers that year, an almost three-fold increase on the organisation’s goal. Alongside recruiting workers, celebrities in the US (where voting in elections is not compulsory) have made a huge difference by calling for people to register to vote. Taylor Swift’s endorsement of Kamala Harris in 2024 saw over 400,000 visitors on the voter registration site within 24 hours of her Instagram post. In 2023, Swift had urged her fans to register, resulting in 35,000 new voter registrations. Similarly, Ariana Grande promoted voter registration in 2019 by setting up booths at concerts on the American leg of her tour, resulting in over 33,000 voters registering. And the same thing is happening closer to home. This year, musician Jack River successfully called for government action to ensure creatives in Australia do not have their work scraped by generative AI companies. Celebrity as the mediator between the punter and the politician Celebrity activism isn’t just a feel-good news story - research tells us it actually makes a difference. By directing attention to issues affecting the average person, like voting inaccessibility or a lack of action on a genocide, researchers Alfred Archer and Amanda Cawston state that “celebrities can play a useful bridging role between political elites and the people they represent”. Alternatively, in a world where talking about politics is “not cool,” or where people lack exposure or knowledge to engage with politics, celebrities can act as an educational resource or an entry point into the political world - when they speak up about an issue, their audience gets exposed to it. Olivia Rodrigo’s Daisy Chain Fields festival is a perfect example. Rodrigo partnered with twelve organisations focused on women’s issues, such as reproductive rights. These organisations had stalls at the festival for people to meet them and were also featured heavily on social media, giving their issues global attention and raising $20 million for the organisations. We have also seen celebrities at award shows such as the Emmys wearing F*CK ICE pins, referring to the U.S. Immigration and Customs Enforcement’s treatment and detainment of people - generally people of colour. When celebrities speak up about political issues in public forums, they put the issue at the forefront of mainstream media, rally their supporters, and make it clear that, however much they disagree, the government cannot simply ignore it. Our over-reliance on celebrities versus our politicians As much as I love seeing my favourite celebrity use their platform for a good cause, there is an important question to ask: not whether celebrity activism is good or bad, but whether we should have to rely on celebrities. And the answer would, in an ideal world, be no. In an ideal world, or merely a just world, action on Palestine would’ve happened decades ago, climate change would be taken seriously, and most importantly, people would understand their democratic rights. However, that is not our reality. Only around 28 per cent of Australian Year 10 students reach proficiency in civics and citizenship. In the US, almost two-thirds of people under 30 say high school civics education didn’t prepare them to understand voting deadlines, how to research candidates, or how to vote by mail. When civics education is weak, our trust in these institutions and our understanding of change-making processes also decline. This is where celebrity steps in and becomes our main source of reliance. As Mary R. Connelly notes in _The Tartan, “_Celebrities will always be involved in campaigns in one form or another, but their presence is not what causes people to vote blindly. US Stand Up for Science movement expands to Europe
A grassroots movement by scientists in the US pushing back against threats to academic freedom is setting up a Swiss outpost to take the movement global. Stand Up for Science started in February 2025 to protest against politically-motivated grant cancellations by the Trump administration. Since then, it has organised numerous rallies by scientists and turned into a broader campaigning organisation, supporting “pro-science” political candidates and attempting to impeach Robert F. Kennedy Jr, the health and human services secretary the group accuses of advancing “quack” science. Now, Stand Up for Science is establishing a Swiss office to take some of its campaigning beyond the US, and its leader is urging European academics to organise similar pro-science groups in case they face similar threats in Europe. There will likely be “spillover” to Europe from what the Trump administration is attempting in the US, said Colette Delawalla, chief executive of Stand Up for Science, who last month returned from a tour of Europe. “Are we going to see a Trump-style scientific [. . .] take over in the same way?” she asked.Delawalla, who is doing a PhD in clinical psychology, said she was driven to help found the organisation after the Trump administration started cancelling health research grants in politically taboo topics, such as transgender people, diversity and HIV, in early 2025. Since then, the administration has introduced several other measures that infringe academic and institutional autonomy.It threatened to cut Harvard University’s federal funding unless the university accepted a “critical mass” of new faculty and students to create “viewpoint diversity.” And this year, it proposed explosive new rules that would allow political appointees at research funders to screen grants to make sure they “advance the president's policy priorities.”US scientists have been at times stunned by the volume of announcements, and the country’s researchers were relatively unprepared. “We sit in front of the fire hose every day,” said Delawalla. For this reason, European scientists need to prepare now for similar assaults. “If you've got a little bit of extra time to start thinking through, and making those preparations and building community and solidarity,” then European researchers will “start five steps ahead of where we were.” She plans to return to Paris in September to share experiences with concerned French academics. Is Le Pen the same?During her European tour, Delawalla said that French academics were beginning to worry about a National Rally (RN) victory in presidential elections next year. She said there were “chilling similarities” between Trump and Marine Le Pen, the RN’s likely candidate. However, so far, the RN hasn’t proposed anything nearly as radical as the Trump administration has carried out. Its current manifesto is all but silent on university and research policy. Le Pen’s 2022 manifesto bemoaned a lack of French R&D spending, including in fundamental research, and promised to modernise labs and strip away grant bureaucracy. The party’s 2024 European election manifesto explicitly supported the Erasmus+ student exchange programme, scientific exchange and joint European technology projects. However, a Le Pen victory next year could see France drastically cut its contribution to the EU, which would hit the budget for science and innovation. Meanwhile, more restrictive immigration policies could hit university recruitment of faculty and students. Climate science denial?As heatwaves and wildfires have ravaged France, the RN has sought to distance itself from previous comments casting doubt on climate science.In the UK, the radical-right party Reform UK, now second in the polls, denied climate research in a draft of its 2024 election manifesto, but its leader, Nigel Farage, then appeared to quietly U-turn and said he was “not arguing the science.” By contrast, Trump has repeatedly and explicitly denied climate change science, in 2025 calling it “the greatest con job.” Like the RN, Reform UK has been light on research and university policy. However, it did say in its 2024 manifesto that universities should suffer “heavy financial penalties” for allowing “political bias or cancel culture” on campus, building on laws brought in by a previous Conservative government. Ahead of September state elections in Saxony-Anhalt in Germany, the poll-leading Alternative for Germany has proposed abolishing gender studies and postcolonial studies, and promised money for “critical” climate research and studies of Islam. Starts with rhetoricEven if European radical right parties aren’t currently planning the draconian policy measures of the Trump administration, Delawalla said that academics should not be complacent, and fight even small funding restrictions or anti-science talk from the start. “It starts off with sowing distrust about institutions,” she said. “It starts off with these small things, and then all of a sudden it's there.” There are other, more established science and university advocacy groups in the US aside from Stand Up for Science. But the group stands out because of Delawalla’s refusal to attempt to appear politically bipartisan or conciliatory. “I firmly believe that the US is experiencing fascism,” she said. “We’re not trying to win over the super Maga people,” she said, “and we never have been.” The lesson of the US is that even if universities or other institutions capitulate to government demands, this doesn’t save them from cuts and restrictions. “It doesn’t matter what people do,” she said. Global “pseudoscience”Another aim of Stand Up for Science’s push into Europe is to better coordinate a campaign to protect people in poorer countries from what the group describes as “pseudoscientific exploitation,” trials outside the US used to build a case against things like vaccines. Earlier this year, the group sent an investigator to Guinea-Bissau to investigate a controversial, Danish-run Hepatitis B vaccine project funded by the US Centers for Disease Control and Prevention.The study would have withheld a proven vaccine from newborns, Stand Up for Science argued in a briefing it presented to Congress, risking their health. Significantly, the research group based in Guinea-Bissau, known as Bandim, focuses on the “non-specific effects” of vaccines: impacts on health beyond preventing the target disease. The US ‘golden age’ science strategy: what Europeans need to know The suspicion is that the study was funded as part of an attempt to shop around for results that could help build a case against vaccines in the US, said Delawalla. “This is just code for vaccines cause autism,” she said.Bandim has put out a rebuttal of Stand Up for Science’s criticisms, arguing its work has uncovered a number of vaccine side effects, both positive and negative, which need to be taken into account in public health. However, Congressional Democrats took up the Stand Up for Science cause, and in February, Guinea-Bissau said it had permanently halted the study. WatchdogBut for Delawalla, this is not the end of the fight. Kennedy has made a number of health claims that scientists think lack evidence, such as the idea that seed oils cause inflammation, for example. Delawalla worries that more studies could be launched in poorer countries to fish around for evidence to support this agenda. Stand Up for Science is talking to African science organisations to create a kind of “watchdog” to scrutinise any further trials. “Outsourcing the investigation to some of the most vulnerable children in the world who are desperate [. . .] is fundamentally wrong,” she said.

WHO Member States adopted a resolution to amend the ‘WHO Global Code of Practice on the International Recruitment of Health Personnel’ (the Code) and strengthen its implementation, following Member State-led consultations in the lead up to the Seventy-ninth World Health Assembly. The resolution marks an important step towards the vision that everyone, everywhere can access competent and motivated health and care workers, as a foundation for both universal health coverage and global health security. Key additions to the Code focus on: inclusion of provisions on health personnel recruited internationally for employment as care workers; applicability of the Code recommendations during emergencies; and encouraging co-investment in health systems and health workforce to ensure international recruitment results in proportional benefits for source and destination countries.Since its adoption by the Sixty-third World Health Assembly in 2010, the Code has undergone three rounds of review of its relevance and effectiveness. “We welcome the resolution’s focus on sustainable and ethical mobility of health personnel,” explained Dr Yukiko Nakatani, WHO Assistant Director-General, Health Systems, Access and Data. “The Code’s adoption in 2010 was a landmark achievement. Following this update, we encourage all countries to enhance implementation, including by viewing co-investment as a win-win.” The additions were recommended by an Expert Advisory Group appointed by the WHO Director-General. The Expert Advisory Group’s final report noted the progress in health workforce data availability, provision of migrant health worker rights and the embedding of ethical recruitment principles in national policies. It also highlighted areas of possible improvement in the Code implementation, including support for the strengthening of health systems in source countries.The adoption of the resolution follows extensive consultations led by Member States, underscoring the continued relevance of the Code in a context of increasing international mobility of health workers and growing pressure on health systems resulting from global health emergencies and funding constraints.WHO is already taking forward actions called for in the resolution. An updated version of the ‘WHO health workforce support and safeguards list’, that guides ethical recruitment practices by identifying countries facing the most pressing health workforce challenges, will be published later in 2026. As recommended by the Expert Advisory Group, this will include additional flexibility enabling countries to indicate their willingness to allow active international recruitment, or for Member States not on the list to request tailored support and safeguards. This approach reflects a more nuanced, country-led framework that balances workforce mobility with the need to protect and strengthen health systems in vulnerable settings.“I thank the members of the Expert Advisory Group, the group of Member States who led the resolution, and all Member States for their constructive engagement,” added Dr Nakatani. “Let’s bring this spirit of collaboration to our collective efforts to implement the resolution, strengthen the health workforce and build resilient and equitable health systems.”

Your Thymus and Your Healthspan
Our thymus gland plays a central role in the development or our immune system, specifically for supporting T cell development (how these cells got their name, maturation and differentiation in the thymus) and discriminating between self and foreign, non-self antigen proteins with production of dendritic cells. As we age, our thymus gland shrinks—the process known as involution—with progressive change from spongy to fatty tissue, with loss of functionality. But this process, with respect to timeline and extent, markedly varies from one person to the next. And to make things even more complicated, our thymus gland anatomy and precise location in the chest also is highly variable. So the famous Frank Netter diagrams from the 1960s (such as at left below) don’t capture the remarkable heterogeneity that a random set of 12 chest CT scans (below at right). What I consider as 2 landmark papers in Nature this week (here and here) used AI to quantify health of the thymus in 2 large cohorts, and then correlate the metric to a broad array of health outcomes. In this edition of Ground Truths, I’m going to cover three questions: (1) How was thymus health determined by AI?; (2) How is thymus health linked to key clinical outcomes? and (3) Are there ways we can promote a healthy (aka rejuvenate) thymus gland in our later years? Subscribe 1. How was thymus health determined by AI? This was accomplished by extensive work developing and validating an AI pipeline for 2 stages, the first for localization and segmentation of the thymus bed, and the second to quantify a digital marker, termed the thymic health score. There’s a lot to this algorithmic development, so I won’t go into all the details, but just provide a rudimentary outline of what was done. The 1st stage used supervised learning with 2 radiologists reviewing 2,461 chest CT scans. That work trained a 3D U-Net model to automatically identify the thymic bed’s 3D cropped region using center of mass coordinates, which reached 99.8% accuracy. The 2nd stage used a foundation model that was pre-trained with SwAV (which stands for Swapping Assignment between multiple Views), a self-supervised model, that generated a high-dimensional thymus representation of 4,096 features. That compares to the radiology reductionist and subjective scoring of 0 to 3 where 0 means the thymus is fully degenerated and fatty, and 3 is considered dense, intact glandular soft tissue. Notably, the self-supervised learning (SSL) performance was superior to supervised (AUC ~0.75 vs 0.55, respectively) reflecting the shallowness of the 0-3 classifier vs SSL’s holistic, self-taught AI. The thymic health score ranged from 0 to 100, with the highest number indexed to fully preserved thymic health. The algorithmic work extended to explainability with both Shapely value distribution, meaning the model was holistic not relying on any “magic” pixel, and occlusion sensitivity, demonstrating the model’s performance was not affected by ribs, sternum, lung tissue, but rather focused on the thymic bed directly. Below are a few saliency maps from the occlusion sensitivity that indicate the thymic bed focus, not affected by neighboring structures, using the jet color scale. Now the AI was ready for processing ~25,000 CT scans, each over 30 MB files, from the 2 large participant cohorts to provide a thymic health score, it did so in less than 14 hours, which is less than 2 seconds per scan! Share 2. How is thymus health linked to key clinical outcomes? The two cohorts were the National Lung Screening Trial, NLST (N=25,031) and the Framingham Heart Study, FHS (N=2,581) each with baseline demographics and long term follow up of >12 years for health outcomes. From the NSLT, you can see below that increased age and body-mass index were correlated to reduced thymic health. And, overall, men had reduced thymic health scores compared with women. There are a lot of graphs in the paper for outcomes, for both the NLST and FHS cohorts. To simplify the major outcomes, below is a composite Figure with striking reduction of all-cause mortality and for the different causes of death. The results were consistent between the 2 cohorts. For example, the cardiovascular mortality hazard ratio average was 0.57 in NLST and 0.38 in FHS (weighted to be 0.53 for the aggregate). The reduction of mortality also extended to digestive diseases and pulmonary disease (data not shown below). Lung cancer incidence was reduced by 36% for high vs low thymic health (Figure below, with a panel -incidence and b panel-mortality, adjusted for age, sex, BMI, smoking status). Notably, there was a smoker’s paradox: lower incidence and improved survival for lung cancer in smokers who had a high thymus health score. Smoking was associated with lower thymic scores whereas alcohol was not. Higher thymic health was linked to higher HDL cholesterol, lower triglycerides, lower fasting blood glucose, and lower systolic and diastolic blood pressure. Inflammation biomarkers, such as C-reactive protein and interleukin-6, were elevated in the people with lower thymic health scores. In their second paper, the same team looked at the relationship between thymic score and outcomes for 3,476 patients receiving cancer immunotherapy. The non-small cell lung cancer progression-free survival was reduced in patients with low thymic health score. A parallel relationship for survival was seen for other types of cancer including melanoma, breast, and kidney with a 44% lower risk among high thymic scores (Figure). A similar pattern was seen with different immune checkpoint inhibitors. The thymus health score outperformed programmed death ligand-1 (PD-L1)and tumor mutation burden (TMB) assay of the tumor and was an independent prognostic marker of progression-free survival. In this dataset, a direct connect with thymus score and adaptive immune function was noted with a correlation before cancer treatment for both T-cell diversity and thymus T-cell production (a metric known as T cell receptor excision circles). Thymus Removal Study There is a highly relevant citation about thymus function before moving onto ways to rejuvenate it. In 2023, a very important study of thymus removal during cardiothoracic surgery was published, with 1146 patients undergoing thymus removal vs 1146 matched controls (mean age 55 years). The adverse outcomes for thymus removal were striking, with a 2.9 higher risk of all-cause mortality and doubling of cancer incidence, 1.5 fold increase in cancer mortality, and 1.5-fold increase in autoimmune diseases. The immune function showed marked compromise after thymus removal, as reflected by CD4+ and CD8+ T cells (Figure below) Share Ground Truths 3. Are there ways we can promote a healthy (aka rejuvenate) thymus gland in our later years? In recent months we’ve learned a lot about the process of thymus involution and the pathways by which this may be modulated. Thymus involution is primarily due to loss to thymus epithelial cells (TECs) and there are 2 subtypes with different functions: the cortical, responsible for positive selection of T cells, and the medullary, for negative selection. When adipose tissue infiltrates the thymus, thymic adipocytes are pro-inflammatory, knocking out T-cell output. Likewise, systemic inflammation, or “inflammaging,” from smoking, obesity, and chronic stress promote thymic adipocytes. Now, from preclinical studies, we know about key pathways that account for these processes. Historically, back in 2014, FOXN1 was recognized as a single master transcription factor essential for TEC function, with studies in mice with forced FOXN1 upregulation leading to thymus regeneration. That laid the foundation for potential regrowth of quiescent thymus cells. It took awhile to find pathways to accomplish this goal and to even refute its role as the master regulator. The Liver-Thymus Axis Feng Zhang and his team recently showed that injection of an mRNA vaccine to the liver encoding DLL1, IL-7 and FLT3-L in combination, factors known to support thymopoiesis (Figure), successfully enhanced immune function in aged mice. However, the effect was transient. The FGF21 Story Two reports in 2025 addressed the thymus gland’s production of FGF21, a growth factor (fibroblast growth factor 21) and its control of thymus involution. This growth factor is also produced by the liver, but it doesn’t have impact on the thymus in aged rodent models. In contrast, The liver’s hepatocyte growth factor (HGF) has been shown to reverse senescent TEC structural changes. Growth hormone stimulates production of insulin growth factor-1 (IGF-1) in the liver and the thymus. Two small studies in human participants (TRIIM, for Thymus Regeneration, Immunorestoration, and Insulin Mitigation) of 6 and 50 people, respectively, treated with combined human growth hormone, metformin and DHEA, suggested the potential of slowing epigenetic aging and improving thymus mass and function. But the combination of drugs, small sample, and lack of controls make conclusions murky. Increased thymus FGF21 led to increased CD8 T cells in old mice and extended healthspan and improved physical performance. Building on the previous FOXN1 data, ablation of β-klotho, the obligatory co-receptor for FOXN1, accelerated thymic aging. The benefit to FGF21 was confirmed in a companion paper using an FGF21 knock-in mouse model showing thymus enlargement and increased TECs throughout the lifespan. (schematic Figure below). The accompanying editorial for these 2 papers speculated: “The proposed link between age-associated thymus involution and organismal aging is intriguing, and suggests we might seek to achieve systemic rejuvenation by counteracting thymic involution.” Other Factors Lessons from the Axolotl (Mexican Salamander) In an elegant set of experiments of the axolotl (Figure), known for its complete thymus regeneration after total surgical removal, mediators were identified. Surprisingly FOXN1 was dispensable. But midkine, a growth factor, appeared to be the driver, the initiator of thymus cell growth, survival, and repair. Other components that contributed were Postn+ (periostin-expressing mesenchymal niches) and Ccl19, a chemokine protein from the bloodstream. RANK and RANKL RANK (receptor activator of nuclear factor κB its ligand (RANKL) have been considered key regulators of medullary TECs. In a study of both aged mice and human thymus cell cultures, their key role was confirmed, with multiple benefits of resorting TEC function, recruitment of progenitor cells, and T cell development. Subscribe Summing Up This body of work strongly supports the health of the thymus gland as a critical regulator of human healthspan, not just a correlate or link. The new landmark studies are reinforced by the regenerative biology experimental models that have shown, via an array of mediators, that healthspan of aged mice is at least in part dependent on thymus gland function and its critical role for maintaining adaptive immunity. It sure seems that we’d be better off having an immune reservoir of naive T cells instead of a profile in older adults of exhausted, low quality, memory T cells, with a senescent phenotype. It is striking that we ignored the importance of the thymus gland for many decades. A wake-up call was the results of the thymectomy matched control study reviewed here, with the opposite of high thymic health score improved health outcomes. The convergence of AI and healthspan here is notable. The exceptional and laborious work for developing and validating (no less explaining) the AI to quantify thymus health set the foundation for probing the connect the major health outcomes in two cohorts with extensive follow-up. We’ve never had a way to meaningfully quantify the thymus gland before, so this represents important leverage of supervised, self-supervised, and transformer models. The study wouldn’t have been possible without AI. And it begs the question as to whether there are ways we can maintain our thymus at a high health score. But it’s not so simple to rejuvenate our thymus. There are multiple risks including the induction of autoimmunity, increasing the risk of cancer, and inducing a pro-inflammatory state. For example, factors that increase TEC proliferation could compromise the medulllary negative selection process and leak out auto-reactive (self-attacking) T cells. Thymic cells could be induced to proliferate, but we know, for example, that growth hormone and IGF-1 are associated with an increase risk of cancer. If thymus rejuvenation doesn’t clear out the accumulated senescent cells in older adults, then inflammation within the gland could block the production of TECs. Keeping these risks in mind, we now have identified many ways to keep our thymus healthy as we age that undoubtedly will be tested in clinical trials going forward. Ultimately, the benefit-to-risk tradeoffs will be defined. In the meantime, we have desperately needed an immunome. As I’ve stressed in multiple prior Ground Truths, we have no test in the clinic to assess a patient’s immune status! It was very encouraging for me to get an email from Hugo Aerts, the senior author of the landmark papers in the days after they were published: “I also wanted to say that your Substack on “Why We Need an Immunome” was inspiring for this work. We found it a very compelling perspective and greatly enjoyed reading it.” Just having an assessment of a person’s immune status via a chest CT scan, with millions of these performed per year, might help in guiding the right immunotherapy for cancer (e.g. more intensive, combinations for those with low thymic health scores) and help define people who are increased risk for age-related diseases of cancer, cardiovascular, and neurodegenerative. The main theme of Super Agers is that we now have many layers of data (genetics, proteins, biomarkers, organ clocks that can be integrated via multimodal AI) for us to define a person’s specific risk decades before one of these age-related diseases leads to symptoms, enabling prevention. From the new studies this week, we can add thymus health score as yet another way to understand a person’s risk, a window into their adaptive immune system and healthspan. And perhaps someday we will be able to safely rejuvenate the thymus, or maintain its health through one’s life, and use the thymus health score to monitor. NB: I wrote this post (No A.I.)
